Invokana is associated with an increased risk of lower limb amputation, but it is disputed as to whether or not the medication is directly responsible for this. In trials, all patients who had amputations had long-standing diabetes, while those who had more recently developed the condition did not have an increased rate of amputations during treatment. One of the more widely-accepted theories is that the increased urine output caused by Invokana was causing patients to be chronically dehydrated, which increases risk of amputation. Regardless, patients who already have a history of amputation may not want to buy Invokana; there are numerous alternative options available.

The effects of Invokana on unborn or nursing babies aren't fully understood. In general the medication should be avoided during pregnancy, particularly during the second and third trimester, or if breastfeeding. However, uncontrolled diabetes presents an equal or greater risk to both mother and the unborn; a doctor should be consulted as to the best course of action if pregnant or trying to conceive.
